Continuity of care
Every Elik treatment runs on the referring nephrologist's prescription. Elik does not assume, redirect, or share the patient relationship: your orders set the treatment parameters, the schedule follows your prescription, and a treatment record returns to your office after every visit. Medication and prescription changes route through you. Elik extends your reach into the patient's home; clinical direction stays where it is.
Which patients fit
Patients who want or need home hemodialysis but cannot or will not self-administer it: patients who live alone, patients without a trainable care partner, patients unable to self-cannulate, patients with vision, dexterity, or mobility limitations, and residents of assisted living and skilled nursing facilities, treated inside the building. New starts and in-center transitions are both routine; hospital discharges are prioritized.
How to refer
Send demographics, the insurance face sheet, and current orders or recent records. Partial packets are fine; Elik chases the remainder. Payer note: Original Medicare does not pay for staff-assisted home hemodialysis, by law. Medicare Advantage plans follow Medicare's reimbursement rules, so they do not pay for it either. Elik provides staff-assisted home hemodialysis for eligible patients with BCBS, Cigna, or UnitedHealthcare/UHC commercial coverage, including when Medicare is the primary insurance. Medicare-only, Medicare Advantage (including BCBS, Cigna, or UHC Medicare Advantage plans), Medicare supplement (Medigap) plans, and Medicaid do not pay for this service through Elik. Send the insurance face sheet with the referral and we give you a same-day payable-or-not answer.
Once the benefit confirms and orders arrive, most patients start within days. Your office receives the start date, the treatment schedule, and documentation after every visit from the first treatment forward.
