What this website collects

The call-back form. This website has one call-back form, on the contact page and the request-a-call-back page, in English and Spanish. It asks for your first name, your phone number, your metro, and the patient's insurance, and lets you add the best time to call or a short question. That is all the form asks for and all it collects. It goes to our admissions team so someone can call you back.

You do not need to use the form. You can call either office instead and send us nothing through this website at all.

Call tracking. The phone numbers shown on this site are routed through a call tracking service before they reach us. It records which number was dialed, which page you were on, and how you arrived at the site, so we know which parts of the website are useful. It places a cookie in your browser to do this. It does not record what you say on the call.

Site analytics. We use a privacy-focused analytics service that counts page views and shows us which pages people read. It does not use cookies and it does not identify you.

What we do not collect. This website has no login, no account, no patient portal, and no place to enter medical information. Do not send medical details through the form. Call us instead.

Who else touches it

A small number of companies handle information on our behalf so this website works: the company that hosts the site, the company that delivers form submissions to us as email, the company that provides call tracking, and the company that provides our email system. They are permitted to use your information only to provide that service to us.

We will also share information if the law requires it.

Your choices

You can ask us what information we hold about you, ask us to correct it, or ask us to delete it. Call the office for your metro and say you are calling about a website privacy request. We will tell you what we have and take it out if you want it out.

Most browsers let you block or delete cookies in their settings. Blocking them will not stop the website from working.

Changes

If we change how this website handles information, we will update this page and change the date at the top.
